Living Conditions for Ambulatory Impaired Individuals
Ambulatory impaired individuals can receive assistance in adapting their apartment and the access routes to the apartment.

Improvement of the living conditions of ambulatory impaired individuals is facilitated via a joint committee of the Ministry of Health and the Ministry of Housing.
Available Adaptations :
** The committee assists in paying for interior and exterior modifications of the dwelling
** Interior modifications – adjustment of the interior of the dwelling to accommodate a al
** Exterior modifications – adaptation of the access routes to the requester’s dwelling, such as : access path, ramp, banisters and elevators.
** Installation of an elevator – for disabled, wheelchair-bound individuals.
Assistance is provided to :
Individuals living in one of the following options, and who meet the eligibility criteria as detailed below.
** In an apartment they own
** In a key-money apartment
** In a public-housing rental apartment
** In an apartment on a kibbutz or in a moshav
Eligible for assistance :
** Anyone whose mobility and.or functions have been impaired as a result of a damage to their motoric system, a patient in the ** advanced stages of an illness (wheelchair-bound, walking with prosthetic limbs, assisted by instruments, crutches etc) (“the eligible individual”, hereinafter).
** The impairment shall be determined by a recognized medical institution : hospitals, the HMOs and government rehabilitative institutions.
The process :
** The social worker in the Departments of Chronic Illness in the Ministry of Health must be approached.
** The HMO, to which the applicant belongs, must be contacted in order to coordinate a visit from an occupational therapist or from a qualified rehabilitation worker for this matter.
** When the relevant documents detailed below are produced, the request shall be forwarded to the Ministry of Housing by the social worker at the health bureau.
Required Documents :
** Request for Assistance form – a signed, filled out form from the applicant and/or the spouse, in two original copies. This form can be obtained from the social worker at the health bureau.
** Medical functional form – a signed, filled form by the doctor and nurse at the medical institution at which the applicant is being treated.
** Psycho-social assessment – shall be performed by the social worker at the health bureau, or by a social worker within other rehabilitation and welfare frameworks, at which the applicant is being treated.
