Who Can Get Vaccinated Right Now?

** All individuals 12 years of age and older that reside in the United States are eligible for the COVID-19 vaccine.
** Some sites are offering walk-up vaccinations for any eligible New Yorkers without an appointment.
** Find a walk-up vaccination site for eligible New Yorkers

Note:
** People who are 12 to 17 years old (minors) are only eligible for the Pfizer vaccine.
** A parent or guardian must provide consent for their child to be vaccinated in person or by phone.
** Some providers, including all City-run sites, will accept proof of consent in writing. However, in-person or phone consent is preferred.
** All minors who are 12 to 15 must be accompanied to the vaccination site by a parent or guardian, or another adult caregiver designated by the parent/guardian.
** The child must also have a completed NYS COVID-19 Immunization Screening and Consent Form .

How Can Homebound New Yorkers Schedule A Vaccination?
Eligible homebound New Yorkers can fill out the Interest Form for In-Home Vaccinations for Fully Homebound New York City Residents or call 877-VAX-4-NYC (877-829-4692) to be contacted by the City for this service.

How do I get a reasonable accommodation to schedule an appointment or travel to a vaccination site?
People with disabilities can get help making a vaccination appointment at an accessible site or traveling to their appointment and getting their shot. This kind of help is called a reasonable accommodation.

You can get a reasonable accommodation if you have difficulty with:
** Seeing or hearing
** Thinking or concentrating
** Speaking
** Using your arms
** Taking care of daily chores
** Coping with feelings of sadness or anxiety
** Getting around or climbing stairs

You can request a reasonable accommodation at a City-run vaccine site through the City’s online appointment scheduler or by calling 877-VAX-4NYC (877-829-4692). You can also ask for a reasonable accommodation from staff at a City-run vaccination site.

For NYC residents who cannot leave their home, you can sign up for an in-home vaccination online or by calling 877-VAX-4NYC (877-829-4692).

What should I do before I go to a vaccination site?
** Most sites are offering vaccinations by appointment only, but several sites are now accepting walk-ups for eligible New Yorkers.
** Wear a face covering to your appointment. You will not be admitted to the site without one.
** You will also need to complete the NYS COVID-19 Vaccine Form before your appointment. The person administering the vaccine is required to check that you completed the form before you can get vaccinated.
** Reschedule your appointment if you are not feeling well on the day of your appointment.

What should I bring to my appointment to prove I am eligible for a vaccine?
** Most vaccination sites will require proof of identity and age. Some sites may require residence in a specific borough or ZIP code.
** Proof of identity and age may include one of the following: Driver’s license or non-driver ID; IDNYC; Birth certificate issued by a state or local government; Current U.S passport or valid foreign passport; Permanent resident card; Certificate of Naturalization or Citizenship; Life insurance policy with birthdate; Marriage certificate with birthdate.

Note:
Minors must present identification to verify that they are at least 12 years of age or have a parent present to attest on their behalf.

How can I get to my vaccination site?
** If you are 65 or older, cannot use public or private transportation and don’t have anyone to take you to your vaccination appointment, other transportation options are available.
** If you use Access-a-Ride or Medicaid-provided transportation, you can schedule rides as you normally would. If you are homebound and 65 or older, you can schedule free ambulette transportation to your vaccination site.
** If you are 65 or older, and do not use Access-a-Ride or Medicaid-provided transportation, you can schedule a free ride to your vaccination site. Your appointment confirmation e-mail from City-run sites will include more information on how to secure transport.

When do I get my second dose of the Pfizer or Moderna vaccine?
The Pfizer and Moderna vaccines require two shots. You should get your second Pfizer shot 21 to 42 days after the first one, or your second Moderna shot 28 to 42 days after the first one. You should get the second dose no matter how much time has passed.

Why should I get vaccinated?
The authorized vaccines are safe and very effective at preventing severe COVID-19 illness, hospitalization, and death. By getting vaccinated, you are helping protect yourself and helping make your community safer.

Do I need to keep taking precautions after I am vaccinated?
Yes! Even after you are vaccinated, you will still need to practice these important COVID-19 prevention steps: staying home if you are sick, washing your hands regularly, wearing a face covering and keeping physical distance from others.
