You will need your date of birth and a mobile number or email address so we can confirm your identity.

Please note that if you cancel your first vaccination appointment, your second appointment will also be cancelled. This is because your appointments are made ten weeks apart and they are linked. When you are ready, you can use the online booking system to rebook your vaccination.
If you want to cancel your second appointment you will need to call the vaccination centre where you had your first appointment to rebook.
Note :
You can also cancel your vaccination by telephone on 0300 200 7813, Monday to Friday, from 8.00 am to 8.00 pm.

FAQs
1. Can I get a vaccine if I am part of a COVID-19 trial?
If you are, or have been, in a COVID-19 vaccine trial, please bring a letter or email to your appointment from the trial team saying that you can be safely vaccinated.
If you are part of any other trial this will not prevent you from receiving your vaccine. If you are taking part in a COVID-19 survey can also receive the COVID-19 vaccination.
2. How can I spot a scam?
The COVID-19 vaccine is free of charge on the health service. The health service will never ask you to send:
** Your bank account or card details
** Your pin or banking password
** Copies of personal documents to prove your identity such as your passport, driving licence, bills or pay slips
If you think you have been a victim of fraud or identify theft, report it to Action Fraud on 0300 123 2040.
National Cyber Security Centre also runs a suspicious email reporting service, so if you get a suspicious email send it to report [AT] phishing.gov.uk If you get a suspicious text, forward it to 7726 and it will be investigated
