How to Book?

Steps :
Step 1 : Visit the official website through provided above.
Step 2 : Next, Select your Age Group

Step 3 : Read the instructions carefully and click on the “Start Booking” button.
Step 4 : After that, Agree the terms & conditions and click on “Next” button.
Step 5 : Fill the application form with the required details.
Step 6 : Finally click on “Submit” button to complete your registration.

Instructions

** If you (or your carer(s)*) are recovered from COVID-19, please do not make your appointment?Please bring your (or your carer(s)*) recovery/discharge certificate and go directly to the government designated vaccination center. Please click to visit the information webpage for more details.

** If you (or your carer(s)*) have history of anaphylaxis, history of reactions to multiple classes of drugs, or history of severe reactions to drugs, other allergens or any vaccines, please do not make your appointment and consult your family doctor or specialist in immunology and allergy.

** If you (or your carer(s)*) (1) have chronic illnesses or (2) is doubtful about the suitability for vaccination due to your medical illnesses, please do not make your appointment and consult your family doctor.

*Elderly aged 70 or above may also bring along with two carers at most to receive vaccine together.

Under the COVID-19 Vaccination Programme, individuals are required to receive two doses of the same vaccine. The second dose of vaccine should be received 28 days after receipt of the first dose to effectively reduce the risk of infection.

** You (or your carer(s)) can choose the place and the date/time for receiving each dose. If you have never received COVID-19 vaccine before, you have to make an appointment for both the first and second doses in this system.

** If you (or your carer(s)) have received the first dose of Sinovac [CoronaVac] vaccine, you can make an appointment for the second dose in this system. You (or your carer(s)) can choose the place and the date/time for receiving second dose.

** Quotas are allocated on a first-come-first-served basis.
** You (or your carer(s)) may also enquire about your application result through this system on or before the date of booking.

** If you (or your carer(s)) want to know more information such as the suitability, efficacy, side effects and the potential adverse reactions of vaccination, please visit the website at www.covidvaccine.gov.hk.

** Before making an appointment, please read Sinovac [CoronaVac] vaccine instructions for vaccination.
** Citizens going to the vaccination centres should also comply with the visitor infection prevention measures of the individual venue.

Requirement

Information Required for Appointment Booking
** Applicant’s Hong Kong identity document number and date of birth.
** A local SMS-enabled mobile phone number.
** Carer (if applicable)’s Hong Kong identity document number, date of birth and local SMS-enabled mobile phone number.

Note :
In order to ensure smooth operation of the Booking System, applicant is required to complete a booking within 30 minutes, after which the booking session will expire and all data entered will not be saved.
