What is HSS NWT Pharmacist Licence?
Applicants may be exempted from having PEBC if, in the province in which the applicant is registered, he or she is not required to hold PEBC. Licensing jurisdiction must mail Certificate of Standing directly to the NWT Professional Licensing Registrar. Three professional references mailed directly to the Registrar, Professional Licensing from the Referees on the reference form found in the application package. Photocopy of birth certificate or citizenship document (if Canadian citizen), or valid immigration or work permit (if not a Canadian citizen).

Requirements For Pharmacist Registration
The Requirements For Pharmacist Licence Registration are given below,
1. Completed application form, with photograph attached.
2. Specified location of work and date of commencement of practice (on application form).
3. Copy of pharmacy degree diploma. If the degree is not in English or French provide a translation.
4. Copy of Pharmacy Examining Board of Canada (PEBC) certificate.
Note:
Applicants may be exempted from having PEBC if, in the province in which the applicant is registered, he or she is not required to hold PEBC.
5. Current detailed C.V/ Resume.
6. Certificates of Standing from jurisdictions where the applicant is (and has been previously) licensed, sent directly from the licensing authority to the Registrar’s office. Unrestricted licence in a Canadian province is required.
7. Three professional references mailed directly to the Registrar from the Referees on the reference form provided.
8. Photocopy of birth certificate or citizenship document (if Canadian citizen), or valid immigration or work permit (if not a Canadian citizen).
** Failure to forward documents as stated above will delay and possibly prevent licensing.
** Allow six weeks from when Registrar receives documentation to when licensing can be expected.
