Requirements For Professional Corporation

The Requirements For Pharmacist Licence Registration are given below,
1. A completed Registration and Permit Application for Health Professional Corporations.
2. Copy of Certificate of Incorporation OR Certificate of Registration as an ExtraTerritorial Corporation.
3. Copy of Articles of Incorporation.
4. Names and addresses of all Directors.
5. Names and addresses of all voting shareholders.
6. Names and addresses of all non-voting shareholders, including their relationship to the applicant.
7. Confirmation of adequate liability insurance for the corporation and its employees.

What You Need To Provide Professional Corporation?

** Completed Registration and Permit Application for NWT Health Professional Corporations.
** Copy of Certificate of Incorporation or Certificate of Registration as an Extra-Territorial Corporation.
** Copy of Articles of Incorporation.
** Certificate of Compliance under the NWT Business Corporations Act from Corporate Registries with the Government of the NWT, Department of Justice
** Names and address of all Directors.
** Names and addresses of all voting shareholders.
** Names and addresses of all non-voting shareholders, including their relationship to the applicant.
** Confirmation of adequate liability insurance for the corporation and its employees.
** Credit card authorization or cheque or money order made payable to Government of Northwest Territories.

Note:
A professional corporation permit expires on the same date as the member’s annual license.

Renew Professional Corporation Registration

What you need to provide
** Completed NWT Renewal Application Form for NWT Professional Corporations before March 1st.
** Credit card authorization or cheque or money order made payable to Government of Northwest Territories.

Note: Registrar, Professional Licensing will email Renewal Application Form for NWT Professional Corporations in January. Licence and other correspondence will be sent to mailing address unless advised otherwise.
