Requirements For Opthalmic Licence

The Requirements For Opthalmic Licence are given below,
1. Completed Application.
2. Copy of valid Certificate issued by the Joint Commission on Allied Health Personnel in Ophthalmology indicating the level of clinical training, broken out as follows,
a) Part I – Ophthalmic Assistant
b) Part II – Ophthalmic Technician
c) Part III – Ophthalmic Technologist

3. Two (2) current letters of character reference.
4. Registration and license fee as indicated above. Attach cheque, money order or Visa Authorization, payable to Government of the NWT for the applicable amount.
5. Letter from employer (licensed NWT Ophthalmologist) indicating location and proposed dates of practice in the N.W.T.
6. Photocopy of proof of citizenship (Canadian birth certificate or valid immigration document).

Note:

1. Submit completed application, along with requirements, to the Registrar at the address above. All licenses are valid until December 31st following date of issue.

2. This personal information is being collected under the authority of the Ophthalmic Medical Assistants Actof the NWT and will be used to process Application for Registration. It is protected by the privacy provisions of the Access to Information and Protection of Privacy Act of the NWT. If you have any questions about the collection, contact the Registrar’s Office

Renew Your Opthalmic Licence

What you need to provide:
** Completed NWT Licence Renewal Application Form on or before December 1st.
** Credit card authorization or cheque or money order made payable to Government of Northwest Territories.

Note: Registrar, Professional Licensing will email NWT Licence Renewal Form in October of each year. Licence and other correspondence will be sent to mailing address unless advised otherwise.
