Notification & Late Registration of Births :

Requirements:
The following documents are required:
** Proof of birth (Child’s health passport)
** The original or certified copy of the ID card and birth certificate for one or both parents.

If you are a foreigner living in Namibia, bring your passport and the permit giving you permission to be in Namibia. It is best that both parents come in person. If one of the parents has died, you should bring also bring a certified copy of the death certificate.
** If the parents of the child are married to each other: Original or certified copy of the marriage certificate or statement from the village headman/woman (traditional leader) if the parents are married under customary law.

If you have lost your own documents, you should first apply for replacement copies at the nearest Ministry of Home Affairs and Immigration offices and then register the birth of your child.

Procedure:
** Parent(s) should complete a Birth Notification Form.
** The Birth Notification Form should be submitted with any other required documents at any Ministry of Home Affairs and Immigration office were birth registrations take place.
** The Ministry will issue a birth certificate if all the requirements were met and the Birth Notification Form was complete correctly.

Births registered offices:
** Child is younger than 1 year: Hospital, sub regional MHAI office, and some magistrate’s courts
** Child is older than 1 year: Regional MHAI office

Correction (Spelling Mistake):
** Correction Form
** Original Birth Certificate
** Documents to act as proof of requested change issuance

Change of Parent(s) Particulars:
** Change of Parent Particulars Form
** Original Birth Certificate
** Certified copy of parent(s) ID or Birth Certificate
** Affidavit stating parental consent for change (from each parent on, or to be added to, the Birth Certificate)

Re-registration (Children under 16 years)
** Reregistration Form
** Original Birth Certificate
** Certified copy of parent(s) ID or Birth Certificate
** Certified copy of parent’s Marriage Certificate (If married)
** Affidavit (from each parent involved) stating parental consent for change

Change of Surname:
(Applicant possess Namibian ID and wants to change details on their Birth Certificate that will also require a change of details on a Namibian ID)
** Change of Surname Forms
** Police Clearance
** Original Birth Certificate (including dependents, if included in application)
** Certified Copy of ID
** Affidavit with Declaration Statement as to why you are requesting a change of surname
** Objection letters from Justice (2 inserts in Government Gazette)
** Adverts in 2 newspapers, 2 times

Submission for Change of Surname:
(Applicant does not possess Namibian ID, or wishes to change Birth Certificate to match their ID)
** Submission for Change of Surname Form
** Original Birth Certificate (including dependents, if included in application)
** Document to act as proof of requested change
** Affidavit with Declaration Statement for adults
** Parent’s ID or Birth Certificate for minors (Original and certified copy)
** Affidavit stating parental consent for minors if parents are applying on their behalf
** Fingerprints
