How to Register at OACEIS Philippines?
To Register for Online Application for Certification and Exemption Information System OACEIS Follow the steps given below,
** Access the application using this link Go to https://oaceis.ddb.gov.ph using your preferred web browser
STEP 1: REGISTER your company details.
Click the register link on the left side of the menu. Input the company details.
Note: Make sure to use an active and accessible email address (company email is advisable). Duplicate company name is not allowed.
STEP 2: Fill out the registration form with your complete company details.
Part I of registration Form: Company Name
Part II of registration Form: Company Address
Part III of registration Form: Warehouse Address
Part IV of registration Form: Company Contact Details
Part V of registration Form: Name and details of Company Head
Part VI of registration Form: Name and details of Authorized Signatory
Part VII of registration Form: Terms and Condition
STEP 3: Check the reCAPTHCA button for further verification. Then Click the Submit Button once finished.
Note: If you wish to cancel your application, click the cancel button.
STEP 4: Open the E-MAIL you provided on the system. You will receive the confirmation email which includes the User’s username and password.
STEP 5: Go back to OACEIS site and from the main menu, click Sign In.
STEP 6: Input the Username and Password sent through E-Mail to gain access to OACIES, and then click the OK button.
Requirements for OACEIS
** Duly notarized Application Form (AF) (Certification and Exemption)
** Company Profile (CP) (FOR NEW APPLICANT ONLY))
** Scanned original copy / certified true copy of the updated Material Safety Data Sheet or Safety Data Sheet (with complete CAS Number) (MS) and / or Certified true copy of the Certification from the Supplier/s (for products with incomplete CAS) (CS) or Certificate of Analysis (complete assay for products with Controlled Chemicals) (CA)
** Updated PDEA License as required (PL)
** Formulation of the product (FP) (for manufacturers/compounders ONLY)
Procedure for OACEIS
** Register your company by filling out the Registration Form. A default password will be sent to the registered email address and then Sign In by using the default password.
** Fill-out the application form and upload the required documents. Only applications with complete requirements shall be processed.
** The application form, together with the required documents, shall be checked and evaluated by the Legal Affairs Division.
** Once the documents have been evaluated, the system will notify the company to pay the fee of P300.00 (per request) at the DDB Cashier’s Office.
** The Legal Affairs Division shall continue processing the application. Thereafter, the application shall be forwarded to the Executive Director for approval/signature.
** Once the Certificate / Exemption has been approved and signed by the Executive Director, the system will notify the company that the requested document is ready for release. The document shall be claimed by the requesting company from the Records Section through the presentation of the Official Receipt (OR).
