Apply For Death Certificate :
It is a legal requirement for deaths in Tasmania to be registered.

If you wish to apply for a certificate, there are 2 types available:
Standard Death Certificate:
Contains all details of the deceased person.
Extract of Death:
Contains name, sex and, date and place death of only.
If a person dies in Tasmania the death must be registered in this State. The registration of a death is compulsory. The Registry of Births, Deaths and Marriages must receive a death registration form within 14 days of the date of death.

Who can apply for a certificate?:
Historical:
Death records are unrestricted after 25 years.
Family:
** Spouse or registered partner. Documentary evidence of relationship required if person not named on death record.
** Registered parents. Documentary evidence of relationship required if person/s not named on death record.
** Non-parental custodian or guardian. Documentary evidence of relationship required.
** Children. Documentary evidence of relationship required if person not named on death record.
** Brother, sister or next of kin if deceased has no surviving spouse, children or parents. Documentary evidence of relationship required.
Welfare:
** Government or non-government welfare agency acting in interests of next of kin of registered person.
** Access by missing person tracing services, such as Red Cross and Salvation Army, are treated under the Registrar’s discretionary powers.
Benefit:
Insurance or credit company, superannuation or trust fund. To enable payment of benefits, direct or indirect, to next of kin or other beneficiary.
Estate Matters:
Executor, Administrator or Public Trustee. Applicants must name the testator or intestate deceased.
Law Enforcement:
** Police (State or Federal) or National Crime Authority in pursuance of an investigation or preparation of a prosecution or pursuant to a valid warrant or court order.
** Other “law enforcement” agencies may be allowed access on approval by the Registrar.
Consular:
Consular Office or foreign legation of country of origin of the deceased person or relative of the deceased person.
Statutory:
Any government body having right of access under Commonwealth or Tasmanian Legislation. Restricted to formal investigation. Agreements as to how information is to be provided or for what cost may be entered into by the Registrar.
Implied Agent:
Solicitor, conveyancer or funeral director acting for a person entitled to access under “Family Access” or “Access for Estate Matter”. Documentary evidence required.
Other Agent:
Any person with written authority from, or power of attorney of, a person entitled to access under “Family Access” or “Access for Estate Matter”. Documentary evidence required.
What is required to register a death?
** The attending doctor lodges a Medical Certificate of Cause of Death with the Registry of Births, Deaths and Marriages within 48 hours of the death.
** The Funeral Director (or person responsible for disposal of the body) usually completes a Death Registration Statement with assistance from the family.
** In certain cases (car accident, suspicious deaths or deaths at home) the Coroner’s Office are responsible for lodging the Death Registration Statement.
Funeral Directors Online:
The Funeral Directors Online service is for use by registered Funeral Directors only. It allows Funeral Directors to lodge a death registration online. All details are then transferred directly to BDM.
