Apply For Birth Certificate :

The Western Australian Registry registers all births that occur in Western Australia since 1 September 1841. A birth certificate is available from the Registry Office on application and payment of the relevant fee.

Who is eligible to apply for a certificate?:
A parent named on a child’s birth certificate can apply for the certificate. The parent’s identification must be provided with the application.

A person aged 16 years or more can apply for their birth certificate but identification must be provided.

A child under the age of 16 years must have the written consent from a parent named in the certificate and that parent’s identification must be provided.

If you are not eligible to apply for a certificate you must provide the written consent or authority from an eligible person and you must also provide three forms of identification for yourself and that person (see certification requirements and Letter of Authority download on this page).

There is unrestricted access and no identification requirements are needed for births that occurred 100 years ago or more.

Who can apply?:
There are conditions that the Registrar of Births, Deaths and Marriages must consider when granting access to certificates under Part 8 Division 4 of the Births, Deaths and Marriages Registration Act 1998.

In Western Australia, Birth Certificates are available to:
** the person named in the certificate (16 years of age or over)
** either parent (named in the certificate)
** legal guardian (proof of guardianship required)

Identification requirements:
When applying for a certificate in person three forms of identification must be provided. Photocopies must be certified by a qualified person.

When applying by post, three forms of photocopied and certified identification documents must be provided with your application. Do not post original documents.

Certification requirements:
Photocopies of identification will only be accepted if they are certified by a qualified person as being “true copies” of the original documents.

Legalising documents for use in overseas countries:
Note: Some overseas countries require Australian documents to be authenticated. Refer to Smartraveller – Legalising Documents for further information on this requirement.

Phone, fax, email or online applications:
The Registry is unable to accept applications by phone, fax, email or online.

Fees :
The following payment methods are accepted :
** Credit card – MasterCard or Visa only and card number must have 16 digits. Available at the Perth Registry and selected Western Australian courts. No other credit cards or travellers cheques will be accepted.

** Eftpos (debit card) – when paying in person at the Perth Registry or at selected Western Australian courts.
** Cheque or money order – made payable to “Registrar of Births, Deaths and Marriages”.

Note :
** When paying at a Western Australian court, cheques or money orders should be made payable to the “Department of Justice”.
** Cash – when paying in person at the Perth Registry or at a Western Australian court.

** Overseas applicants – Overseas applicants must pay in Australian dollars by quoting credit card details (Visa or MasterCard only) or using an international money order or bank draft made payable to “Registrar of Births, Deaths and Marriages”. No other form of payment can be accepted by the Registry.

** The certificate fee includes regular domestic or overseas airmail postal delivery.

Within Australia :
** Where a priority service fee has been paid, includes priority domestic postal delivery.
** Registered Post or Express Post (only if required by the applicant): Please enclose a self-addressed Registered Post or Express Post envelope with your application.

Overseas customers :
** Priority delivery (only if required by the applicant): You should arrange for a courier service to collect document/s from the Registry at Westralia Square, Level 10, 141 St Georges Terrace, Perth, Western Australia 6000 (8:30am to 4:30pm, Monday to Friday, excluding public holidays). If you are arranging a courier please include this information in your application.

Birth, Death or Marriage Certificate – $49
** Includes a one 10 year search.

Priority Service – $34
** In addition to any other fees.

** Not payable for certificates when lodged in person at the Perth Registry Office or Albany, Armadale, Broome, Bunbury, Busselton, Carnarvon, Esperance, Fremantle, Geraldton, Joondalup, Kalgoorlie, Karratha, Katanning, Kununurra, Mandurah, Manjimup, Merredin, Midland, Northam, Rockingham and South Hedland Courts).

Commemorative Birth Certificate – $59
** Includes a one 10 year search and a standard birth certificate.

Birth, Death or Marriage Certificate – $35
** Reduced fee – event at least 75 years old.

Uncertified copy of a registration – $20
** Births 100 years or more, Marriages 75 years or more & Deaths 1983 back).

Change of Name – $173
** Preparation and registration of a Change of Name. Includes a certificate.

Change to a Child’s given name(s) once within 12 months of birth – $49
** Includes a certificate.

Replacement Change of Name Certificate (1957 onwards) – $49
Correction of Entry – $49
** Addition to, or correction of, information in the register. Includes a replacement certificate.

Search of an index – $34
** Includes a search over any period not exceeding 10 years and a Result of Search Certificate.

Marriage :
Celebration of marriage (including GST) – $204.

Notice of intended marriage (including GST) – $167 (not refundable).
Shortening of time – $42.
