Replacing Lost/ Damaged Driver’s Licence :
Lost or damaged driver’s licences can be replaced by:
** Submitting an application form.
** Providing supporting identity documents.

** Paying the application fee.
This also applies if you are replacing your licence for other reasons, such as when you have added a new vehicle class to your license .
Replacing a stolen driver’s licence:
If your licence has been stolen, you do not need to pay the required fee. However, you must file a Western Australian (WA) Police report.
Please provide the police report number in your application to waive the application fee.
Step 1: Complete application form
Please download the form DL 26 to apply for a replacement driver’s licence.
Step 2: Provide proof of identity
You will need to provide acceptable primary and secondary proof of identification. One of these must show your signature.
If you are applying by post, you will need to send us certified copies of your identity documents. Please contact us to confirm we hold a valid photo for you before you apply by post.
If you are applying in person, you may bring originals to be witnessed by a Department of Transport employee.

Step 3: Get your picture taken (if required)
You will need a photograph for your replacement driver’s licence.
If your photograph and signature held on file with the Department of Transport is up to date (within 10 years), you will not need to do anything else.
If your photograph is not in our records, or is more than 10 years old, you will need to have your picture taken at a Driver and Vehicle Services centre or regional agent, or submit a photo kit by post.

Step 4: Submit the application
You can apply for a replacement driver’s licence either by post, or in person.
Note: You can submit an application by post if your photograph and signature are on record with the Department of Transport. If not, you must apply in person or attach a completed photo kit.
Once received, we will process your application and post a replacement driver’s licence to the address that is in our records.
Submitting application by post:
Please post the following documents to us:
** Completed application form DL 26.
** Certified copies of your proof of identity.
** Completed photo kit (if applying from overseas or interstate).
** Police incident report number (if your licence was stolen).
** Application fee (if your licence was not stolen).
Please make cheques or money orders payable to Department of Transport.
Applications should be posted to:

Submitting application in person:
** If your photograph and signature are not recorded with the Department of Transport, or you prefer to submit the application in person, you will need to go to a Driver and Vehicle Services centre or regional agent.
Please bring along the following:
** Completed application form DL 26.
** Original or certified copies of your proof of identity.
** Police incident report number (if your licence was stolen).
** Application fee (if your licence was not reported stolen).
