Process:
** Register to website: qceservices.quezoncity.gov.ph using your personal email address
** Fill out the form properly to register and submit
** Accomplish Sectoral Information. Make sure to fill-out all the details in this section.
** Upload Documentary Requirements for Solo Parents
** The application will be reviewed by Solo Parent approvers/social workers
** Wait for an email to be sent regarding the Solo Parent ID Application. Applicant will be receiving an email to schedule an appointment for social worker’s interview and assessment.
** Make online appointment @ the qceservices.quezoncity.gov.ph website, SSDD page. Present the reference number for verification of the schedule
** Come to SSDD (2nd Floor), Solo Parent Section on your appointment date & time for your interview & assessment
** Once approved the application, install the QCitizen Application in Playstore / Appstore to get the Digital ID.

Documentary Requirements of Solo Parent ID

New Applicant:
** Accomplished Application Form
** 1 pc. Recent 2×2 ID picture (white background)
** Original Barangay Certificate (Solo Parent)
** Birth Certificate of minor child/children (Xerox copy)
** Medical Certificate/PWD ID (if spouse or child/children is PWD)

Any of the following:
** Photo copy of Death Certificate of spouse (if widow/widower)
** Affidavit (Circumstances of Being a Solo Parent)
** Copy of filed /Pending Petition for annulment of Marriage /Legal Separation
** Copy of Certificate of Detention/Court Order (if spouse/partner is in prison)
** DSWD Foster Care license (for Foster Parent)

For Legal Guardian:
** Accomplished Application Form
** Original Barangay Certificate (Solo Parent)
** 1 pc. Recent 2×2 ID picture
** Birth Certificate of minor child/children (Xerox copy)
** Birth Certificate of minor’s parent / guardian (Xerox copy)
** Barangay Blotter
** Affidavit of Guardianship

Any of the following:
** Death Certificate of minor’s parent/s
** Copy of Certificate of Detention/Court Order (if minor’s parent/s is in jail)

Here are some additional things to keep in mind:
** The processing time for Solo Parent ID applications is usually 15 to 30 days.
** There is a processing fee of P100 for the Solo Parent ID.
** The Solo Parent ID is valid for five years.
