India Embassy Loss & Damage Passport

New Passport In Lieu Of Lost Passport:
Requirements:
1. Application in EAP-I form. All particulars in the application should tally with those in the lost passport.

2. Photocopy of the lost passport
3. 5 recent photos with white background and frontal view.(51mm*51mm).
4. Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation.

5. Original with a copy of one or more of the photo-affixed identity cards to establish identity and nationality(The original will be returned after verification):-(a) Identity Card (b) Labour Card (c)Health Card (d) Driving License (e) Election commission card

6. A sworn affidavit duly attested by theConsular officer (Visit www.ivsglobalattestation.com), explaining
a) The circumstances under which the passport was lost;
b) Details of travel on the lost passport;certifying that the applicant has not acquired any other nationality.

c) Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports made by the applicant on the lost passport;certifying that the applicant has not acquired any other nationality.
d) Places of stay and work in UAE since arrival

7. Personal Particulars Form (in Triplicate with pasted photos) in the case of passports issued by other Passport Issuing Authorities
8. Personal appearance of applicant is compulsory to establish physical identity.

Note:
In The Case Of Applicants Who Are On Short Visit To Uae And Not A Resident In This Country, He/She Will Be Issued An Emergency Certificate To Return To India.

Requirements

1. Application in EAP-I form. All particulars including name and signature in the application should tally with those in the lost passport
2. Damaged passport, in original, with a photocopy.
3. 5 recent photo with white background and frontal view.(51mm*51mm).

4. Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage)

5. Original with a copy of one or more of the following photo-affixed identity cards to establish identity and nationality (The original will be returned after verification):-
(a) Identity Card (b) Labour Card (c)Health Card (d) Driving License (e) Election commission card

6. A sworn affidavit duly attested by the Consular officer (Visit https://www.ivsglobalattestation.com/
a) The circumstances leading to the damage of the passport;
b) Details of travel on the damaged passport;

c) Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports made by the applicant on the damaged passport;certifying that the applicant has not acquired any other nationality.
d) Places of stay and work in UAE since arrival

Note:
In The Case Of Applicants Who Are On Casual Visit To Uae And Not A Resident In This Country, He/She Will Be Issued An Emergency Certificate To Return To India.

Passport Will be retained during the Tenure of the Service.
