Birth/Marriage/Divorce/Death Duplicate Certificates :

Who is eligible to receive the duplicate birth, marriage, divorce, establishment of paternity, adoption, change of name/surname, and death certificates?:
** Duplicate certificates are issued to persons, for whom the act of civil status is made;
** Death certificate duplicate is issued to a relative of the deceased or other interested persons;

** Duplicates of certificates of civil status registration for a deceased are issued to a heir at law or a heir by will of the deceased.
** Duplicate of the birth certificate of a minor child is issued to parents, custodians, representatives of a tutorial or trustee institution; in case of majority duplicate of the birth certificate can be issued only to parents and to other persons on the basis of a notarized power of attorney;

** Duplicate certificates of incapable persons are issued to their custodians.
** Duplicate certificates can be issued to other persons on the basis of a notarized power of attorney.

** Duplicate of certificate of servicemen are sent to the commander of the respective military unit. Duplicate certificate of the persons, being in penitentiary departments, are sent to the head of the respective institution.

How to obtain a duplicate of birth, marriage, divorce, establishment of paternity, adoption, change of name/surname, and death certificates? :
To obtain a duplicate of birth, marriage, divorce, establishment of paternity, adoption, change of name/surname, and death certificates, you may refer to Public Service Hall.

You must present following documentation:
Identity document of the applicant (identification card or the passport);
You will be able to receive a Receipt of payment of the state duty and fill in an application form at the Public Service Hall.

Terms of service and the state duty:
On 5th business day: 2 GEL
On 3rd business day:2 GEL – service duty | 5 GEL- the state duty for acceleration
In 24 hours : 2 GEL – service duty | 15 GEL – the state duty for acceleration
On the same day : 2 GEL – service duty | 25 GEL – the state duty for acceleration

Refugees and IPDs, upon submitting document confirming their status, are exempt from the state duty.

Documents to be presented :
An authorized person may apply for registration of birth and submit one of the following documents taking into consideration the registration basis :
** Document on birth of a child issued by medical facility;
** Decision on establishment of a legal fact;

In addition, taking into consideration the basis for indication of parents :
Parents having the registered marriage should submit the following documents for registration of birth :
** Marriage Certificate ;
** Proof of identity documents of parents.

Parents who are not married should submit the following documents for birth registration :
** Proof of identity document of both parents.

Note :
** if one of parents who are not married or both of them is a minor, Birth Certificate of a minor mother and/or father of a child and a written consent of a parent or any other legal representative on indicating a person as a father of a child should be presented.

A mother of a child, who is not married, should present the following document for birth registration :
** Proof of identity document.

Please, take into consideration :
** The documents contemplated thereto must be submitted before authorized authorities in a material form. Submission of documents is not mandatory, if the e-database of Legal Entity of Public Law operating within the governance of Ministry of Justice – Public Services Development Agency – contains the information given in the relevant document.

** Furthermore, it is mandatory that the documents issued (certified) in a different state (except for proof of identity documents) must be submitted in duly legalized or Apostille certified form, with the translation attested in accordance with the Georgian laws unless International Treat or Agreement of Georgia envisages otherwise.
