Issuance of Passport

Information on issuance of passport to citizens of the Republic of Uzbekistan:
In connection with the achievement of 16 years of age :
1. Copy of birth certificate;
2 . Copies of passports of parents sheets on which there are records or notes .

( If you can not provide a copy of the passport of a parent must attach a copy of the document , in particular , death certificates , divorce , help law-enforcement bodies for recognition of this entity is unknown and missing , etc.);
3 . Form number 1 (1 pc. )
4 . Photo – 4 pcs . , Size 35 x 45 mm;
5 . A certified copy of a piece of departure , which is inscribed in the applicant ( for CIS countries) ;

6. If one parent is a foreign national , you must provide a legalized ( for CIS notarized ) statement of the foreign parent’s consent for a child passport of Uzbekistan ;
7. A copy of the registration card to the applicant’s permanent consulate.

In connection with the expiration of the term for incompetence and lack of sheets passports to visas and special marks:
1. A copy of the passport pages on which there are records or mark;
2. Form number 1 (1 pc.)
3. Photo – 4 pcs., Size 35 x 45 mm;
4. Document confirming leaving to relocate – the presence in the passport sticker recording “milking yashashga chikishga ruhsat berildi” issued by MIA RU and for CIS stamp extracts from addresses in the Republic of Uzbekistan and the copy sheet departure.

In connection with the loss:
1. Copy of passport (if available);
2. Form number 1 (1 pc.)
3. Photo – 4 pcs., Size 35 x 45 mm;
4. Statement detailing the circumstances of loss of passport;
5. A completed application form to request the applicant to verify identity;
6. Part of the law enforcement authorities of the host country on request of the applicant about the loss of passport.

In connection with the change of surname, name, patronymic
1. A copy of the passport pages on which there are records or mark;
2. Form number 1 (1 pc.)
3. Photo – 4 pcs., Size 35 x 45 mm;
4. Document confirming leaving to relocate – the presence in the passport sticker recording “milking yashashga chikishga ruhsat berildi” issued by MIA RU and CIS countries – stamp sheet and a copy of the extract of departure.

Note: The photos for a passport must be printed on photo paper quality (production Kodak, Agfa, Konica, Epson, etc.) from the front, without a hat, with the exception of clothing with a high neckline or cleavage.
